How is a medical diagnosis made? - medical diagnosis more condition_symptoms form

I know this is extremely broad. I understand that only a doctor can do. But what standards are generally not hospitals? There are "diagnoses" is usually written on paper with clear guidelines, or is) the experience and knowledge of the physician, the most important (factor?

There are few guidelines in the field of mental health because it tends to be a very subjective science (which is logical, not ...). These guidelines will be in the DSM-IV.

As for the physical diagnosis, which is used unclear or confusing objective observations, a process of elimination to reach a definitive diagnosis. After the diagnosis is only good until another set of objective data and clinical observations were evaluated. Dx If it something more than new elements or findings from other materials, then it is. Nothing should be written in stone on the medical diagnosis.

All health professionals who assisted in NICE Clinical Governance and evidence-based practice of clinical research and large clinical trials.
However, nothing is conclusive is of 100 percent, and errors and misdiagnoses occasionally happen, but if the health professionals follow appropriate protocol, guidelines and training on medical evidence of such errors due eliminated.
Finally, that the golden rule will be communicated to all health professionals that if in doubt or unsure of reference or search for a different opinion of their colleagues and other health professionals with specific skills.
